Most Official Bankruptcy Forms are scheduled to be replaced with substantially revised, reformatted and renumbered versions effective December 1, 2015, if approved by the Judicial Conference at its September 2015 meeting. Find out more about the pending bankruptcy forms.
Official Bankruptcy Forms Official forms are approved by the Judicial Conference of the United States and are mandatory. They must be used to file a bankruptcy petition and take certain other actions in a bankruptcy case, such as filing a Proof of Claim or applying to pay a filing fee in installments.
Procedural Forms Procedural forms are drafted and approved by the Administrative Office of the U.S. Courts. They are recommended for use, rather than mandatory.
Local forms are drafted and approved by the Court for use in the Middle District of Pennsylvania. The Local Mandatory Forms are the Court's Local Bankruptcy Forms. They are effective January 1, 2005 and were modified on September 1, 2014.
