FORM CHANGES EFFECTIVE DECEMBER 1, 2015:
Most Official Bankruptcy Forms have been replaced with substantially revised, reformatted and renumbered versions effective December 1, 2015. These new forms must be used in all cases filed on or after December 1, 2015. The use of the new forms in cases filed before December 1, 2015 is encouraged to the extent just and practicable.
Official Bankruptcy Forms Official forms are approved by the Judicial Conference of the United States and are mandatory. They must be used to file a bankruptcy petition and take certain other actions in a bankruptcy case, such as filing a Proof of Claim or applying to pay a filing fee in installments.
Local forms are drafted and approved by the Court for use in the Middle District of Pennsylvania.
