Rule 3006-1 Claims - Withdrawal.

A request to withdraw a claim must state:

    (a)    whether an objection to the claim was filed;

    (b)    whether a complaint was filed against the claimant; and

    (c)    whether the claimant has accepted or rejected a plan or otherwise participated significantly in the case.
