Overview:
At any time after the confirmation of the plan but before the completion of payments under such plan, the plan may be modified upon request of the debtor, the trustee or the holder of an allowed unsecured claim. 

An amended post confirmation plan and a passive notice containing the objection date must be provided to each creditor and party in interest, either by U.S. mail or electronically through the CM/ECF system. 

This filing should be accompanied by the Motion, Amended Plan, Notice, and a Proposed Order.  When required by Local Bankruptcy Rule 9013-3, a Certificate of Service must also be filed.
