Overview:
Plans in Chapter 11 individual debtor cases may be modified at any time after confirmation of the plan but before the completion of payments under the plan, whether or not the plan has been substantially consummated.

In a Chapter 11 case in which the debtor is not an individual, after confirmation of a plan, only the proponent of the plan or the reorganized debtor may seek to modify the plan. In a Chapter 11 case in which the debtor is not an individual, a request for post confirmation modification must be made prior to substantial consummation of the plan.

 A modification is effective only if the court, after notice and a hearing, confirms the plan as modified.

This filing should be accompanied by the Motion and a Proposed Order.  When required by Local Bankruptcy Rule 9013-3, a Certificate of Service must also be filed.
