Overview:
In a chapter 7 liquidation, chapter 12 family farmer, or a chapter 13 individual case, a proof of claim is timely filed if it is filed not later than 90 days after the first date set for the meeting of creditors. Governmental units have a 180 days after the date of the order for relief to file claims. For limited grounds the court may extend the time to file proofs of claim in these chapters. In a chapter 11 case, the court shall fix the date and for cause may extend the time.

This filing should be accompanied by the Motion, Notice of Motion, and a Proposed Order.  When required by Local Bankruptcy Rule 9013-3, a Certificate of Service must also be filed.
